Letter of authorization
The one page letter a plan sponsor sends its TPA to name Soro an authorized vendor.

1. Authorization
The plan sponsor names Soro LLC an authorized vendor and business associate of the plan, effective on the date signed below.
2. Scope
The TPA is authorized to provide Soro with the data and access it needs to audit claims:
- Paid-claims extracts, explanations of benefits, and remittance data
- Itemized bills for claims Soro selects for review
- Direct communication with Soro on claim questions
- Acceptance of claim adjustment requests submitted by Soro on the plan's behalf
3. Limitations
Soro has no discretionary authority over the plan. Soro may not change benefits, eligibility, or cost sharing. Its role is limited to auditing claims and requesting corrections.
4. Duration and revocation
This authorization stays in force until the plan sponsor revokes it in writing.
5. Signature
- Plan sponsor name and plan name
- Authorized signer name and title
- Signature and date
